PRONC’s surrender process begins with filling out this form. Once that is done, we review these forms and get the proper representative in
touch with you to arrange a pick up.We want to be sure that we place the right Pug with the right Forever Home!. So the more we know about your pug the better we can place it in a forever home. The more we know about your pug, the easier it will be for us to find it the prefect adoptive family for him. Tell us about your pug (s) health problems, personal habits, or special needs that we and the new owner should be aware of.

IMPORTANT NOTE: All owner surrender pugs must be current of their Rabies vaccination.
Due to the high threat of Rabies in our area this must be done to protect our representatives as
well as our own pets, as this pug will be fostered inside our homes, with our families. Other vaccines are not mandatory but appreciated.

does he/she know any tricks? Does he/she ride in a car well, anything else you can think of that will help us place this pug in a forever home?

12. Is this animal micro-chipped? If so please tell us the chip number and MFG Brand -
